Hostel Outreach Program
The Hostel Outreach Program (HOP) provides individualized support to men who are homeless or under-housed and are living with a serious mental illness.
COTA Health was selected, by the Mental Health Commission of Canada, to provide a Recovery Oriented Assertive Community Treatment (ACT) Team in Toronto. This is a four year project that started in 2009 and operated until 2013 in Vancouver, Winnipeg, Montreal, Moncton and Toronto.
Streets to Homes Follow-Up Housing Support
COTA Health is one of the community partners in the "Streets to Homes" program, a partnership with the City of Toronto. This program works with street-involved, homeless individuals and its goal is to end street homelessness within the City of Toronto utilizing the Housing First strategy.
