Assertive Community Treatment (ACT) team


Cota’s ACT team provides support to adults (18+ years) who are homeless in the City of Toronto and are living with a serious mental illness as typically determined through a clinical diagnosis or as suspected based on the person's personal experience and report of mental health challenges over time. Based on a Housing First model of service, this multidisciplinary team works with service users to find safe and affordable housing first based on personal preferences and then supports individuals to maintain housing stability and the pursue personal recovery.

Services Offered


Our ACT team's services are committed to being flexible and responsive to the unique needs and desires of each client. The supports offered include:

  •     Working with the client to find housing based on personal preferences
  •     Support in accessing basic necessities such as personal identification, food, clothing, health and income support
  •     Comprehensive biopsychosocial assessment
  •     Collaborative development and implementation of an individualized recovery plan
  •     Outreach supports and the development of a trusting relationship between the client and the service provider team
  •     Assistance to achieve/maintain optimum health and wellness
  •     Link to community resources: social, recreational and/or primary health services
  •     Direct support in the areas of
    •         education about the nature of mental illness
    •         support and education to care givers and/or the person’s support network, as appropriate
    •         development of self-management strategies
    •         encouragement, emotional support and motivation
    •         development of personal support networks
    •         development/maintenance of abilities relating to activities of daily living

The Cota's ACT team's services are available in the City of Toronto.
