Bailey House

Bailey House is a supportive housing program that provides rent-subsidized accommodation and support for 10 men living with mental health challenges and co-occurring diabetes or pre-diabetic conditions. Each tenant has his own bedroom and shares a bathroom and communal kitchen with other tenants.

Services Offered

Cota provides on-site support during the day seven days per week. Security is on-site seven nights per week.

Our staff work collaboratively with tenants, their families, the landlord and other clinicians, as appropriate, to assist tenants to

  •     Develop an individual safety and housing support plan;
  •     Maintain their housing stability and a healthy housing environment;
  •     Acquire and/or maintain the skills necessary for daily living;
  •     Participate in health promotion and community development activities such as tenant meetings, structured educational events and social activities;
  •     Resolve situational problems/disputes;
  •     Link to other desired community services/supports, as needed;
  •     Achieve and maintain optimum health and wellness
Cota provides three meals per day and a snack.

Services Available To

Men who are 16 years of age or older living with serious mental illness who are homeless, or at significant risk of becoming homeless, and have diabetes or a pre-diabetic condition. All tenants must also have ongoing support from an external support worker or case manager.

Applicants must also have some means of financial support such as the Ontario Disability Support Program (ODSP) or Ontario Works (OW).

Referrals for Bailey House are only accepted from the Access Point, please contact 416-979-1994 ext. 261 to make a referral.
