Case Manager, Dual Diagnosis and Mental Health experience

Toronto East IST team 

APRIL 10, 2018 – APRIL 25, 2018 

The Case Manager, Dual Diagnosis provides support to clients living with a dual diagnosis (co-occurring developmental disability and mental illness), as well as clients living with symptoms of a mental illness. The Case Manager also works with clients who have dual diagnosis and justice system involvement. She/he is responsible for engaging with each client, developing goal plans, providing service coordination, coaching, skills teaching, safety planning and linking/referral supports aimed at assisting each client to achieve his/her goals.

Cota seeks a Nurse for their Assertive Community Treatment (ACT) team in Toronto. The “At Home” ACT team participated in the Mental Health Commission of Canada’s national research demonstration project on Housing First and continues to serve homeless individuals with serious mental illness and substance abuse issues. Cota’s ACT team provides housing opportunities for clients using a “housing first” scattered site approach.
