Case Management

Cota’s ABI Case Managers provide practical individualized support in the community to adults 16-64 years of age living with an acquired brain injury across the Toronto and in South York Region.

Cota’s Toronto-based ABI case Managers are members of Cota’s Integrated Service Teams (ISTs) which are multidisciplinary teams that provide case management services to multiple target populations within defined geographic catchment areas. Each Cota IST is comprised of case managers that specialize in Mental Health, Acquired Brain Injury, Substance Use, Dual Diagnosis and Geriatric Mental Health.  The ISTs receive support from consulting psychiatrists.

Cota’s IST services are aligned with the geographic areas of Toronto-based Ontario Health Teams (OHTs), as follows:

  • Cota’s North York IST is aligned with both the North Western Toronto OHT and North York Health Partners OHT
 
  • Cota’s Scarborough IST is aligned with the Scarborough OHT
 
  • Cota’s Toronto West IST is aligned with both the Toronto West OHT and Toronto Mid-WestOHT
 
  • Cota’s Toronto East IST is aligned with the Downtown East OHT and East Toronto Health Partners OHT

This program provides rapid, short-term intervention to address an acute situational crisis for individuals with acquired brain injury. For the purpose of this service ‘situational crisis’ is defined as:

  • A situation where an individual (or their families) usual coping mechanisms are overwhelmed, leading to a disruption in their normal functioning and an inability to resolve the situation using their standard problem-solving resources
  • Is temporary in nature
  • A circumstance, that if not addressed quickly, could lead to potentially harmful outcomes.

Note: this excludes situations that require immediate crisis support services, such as 911 or 211

Key aspects of Cota’s ABI Crisis Case Management services include: rapid assessment, crisis intervention, resource coordination, safety planning, advocacy, follow-up and monitoring.

People eligible for this service:

  • Are between the ages of 16 and 64
  • Reside within the city of Toronto
  • Have a documented acquired brain injury and are experiencing long-term (>6 months) functional and/or psychosocial challenges following initial hospitalization and inpatient treatment/rehabilitation
  • Are willing to participate in, and have consented to, a goal-driven service
  • Have complex support needs (excluding degenerative conditions such as Korsakoff, dementia, or other degenerative conditions)
  • Are not in receipt of insurance funds
  • Do not currently have other case management services in place of a similar nature
  • Have met the definition of ‘situational crisis’, and require a direct visit (face-to-face) minimally once/week


To Make a Referral:

Any caregiver, professional or client can make a referral for Cota ABI Case Management services through the Toronto Acquired Brain Injury (ABI) Network. Please complete the ABI Client Community Profile located on the Toronto ABI Network website at www.abinetwork.ca and submit it. You may also contact the Toronto ABI Network at 416-597-3057. You will be contacted by a Cota employee once your referral is processed.

Supportive Housing

This program provides rent-geared-to-income accommodation within a Toronto Community Housing building in downtown Toronto and dedicated 24/7 on-site Cota support to 20 residents who are living with an acquired brain injury.  The goal of the program is to support residents with activities of daily living and personal goals so that they can successfully maintain their housing and lead fulfilling lives within the community.

Other Programs

*Please note: this program is currently on hold and not accepting referrals* Cota’s Acquired Brain Injury (ABI) Behaviour Therapy Service provides behavioural support and case management services to people who are living with an ABI, are 16 years of age or older and reside within the boundaries of Toronto Central LHIN. The team, which includes a psychologist, behaviour therapists and a case manager, support clients by providing a combination of skills teaching, behavioural approaches and case management supports.

Cota’s ABI Adult Day Service offers a variety of creative group sessions for adults 16-64 years of age living in the City of Toronto. Groups rotate every 12-16 weeks and sessions are available Monday to Friday. The service is located at 50 Fairfax (Door 4A), in Scarborough.
