Cota's Annual Event (2014)

Cota hosted its annual event on June 19, 2014, the Ontario Heritage Centre. At the event, we profiled some of the new innovative community initiatives that we are involved in. Guest speakers will present information about our collaborations with St. Michael’s Hospital, the City of Toronto and Black Creek Community Health Centre.

Vania, Senior Director, Health System Integration, Design and Development from the Toronto Central LHIN opened our annual event. Jeannette, Manager, Mental Health Services at Cota also manages the Coordinated Access to Care from Hospital Emergency Departments (CATCH-ED).

Vicky and Sylvia present about the Mid-East Toronto Health Link Integrated Service Coordination Team: FOCUS. This a new, innovative partnership program between the St. Michael’s Hospital Mental Health Service and Cota.

The new program relies on an interdisciplinary team approach to meet the needs of clients with mental health and complex care needs in Mid-East Toronto.
