Case Management Services

Cota’s Integrated Service Teams (ISTs) are multidisciplinary teams that provide case management services to multiple target populations within defined geographic catchment areas (LHIN sub-regions). The multidisciplinary format of our ISTs allow each case manager to offer service to clients within his/her area of expertise while promoting knowledge exchange and mutual support amongst team members to enable better care to clients presenting with complex needs. Presently, Cota’s ISTs offer services in following LHIN sub-regions:

  • North York IST: North York Central and North York West Sub-Regions
  • Scarborough IST: Scarborough North and Scarborough South Sub-Regions
  • Toronto West IST: Toronto West and Toronto Mid-West Sub-Regions
  • Toronto East IST: Toronto Mid-East and Toronto East Sub-Regions

Services include assessment, individualized recovery/goal planning and direct support in areas of identified need.

Acquired Brain Injury Case Managers

Cota’s ABI Case Managers provide practical individualized support in the community to adults 16-64 years of age living with an acquired brain injury across the City of Toronto and in South York Region.

Addictions Case Managers

Addictions Case Managers provide individualized support to adults (16+ years) living with a serious mental illness and addictions issue in the City of Toronto.

Dual Diagnosis Case Managers

Dual Diagnosis Case Managers provide individualized support to adults (16+ years) living with a co-occurring developmental disability and a diagnosis of serious mental illness and live in the City of Toronto. Clients work collaboratively with a case manager towards achieving identified goals In addition, we provide Dual Diagnosis & Justice Case Management (DD & J CM) for clients with a dual diagnosis who have involvement with the criminal justice system.

Geriatric Mental Health (GMH) Case Managers

GMH Case Managers provides individualized support to clients who are 65 years of age or older with a diagnosis of dementia or serious cognitive decline or be of any age with an early onset diagnosis of dementia. And all eligible clients must also have have complex care needs (for e.g., significant mental health challenges, frailty issues, a serious/chronic medical illness which requires intensive support, significant challenge with maintaining safe living situation, etc.) This service is provided across the City of Toronto.

Mental Health Case Managers

Mental Health Case Managers provide individualized support to adults (16+ years) living with a serious mental illness. In addition, Cota also provides targeted mental health case management support to Homeless or under housed adult men (16+) via our Hostel Outreach Program (HOP), which is delivered by both our Toronto West and Toronto East ISTs. A HOP drop-in is also available to homeless men across the City of Toronto every Friday morning from 10:00pm to 12:30pm, located at Christian Resource Centre (CRC), 40 Oak St., in the North East corner of the main dining area. Clients have the option of having a full lunch from 12-1pm.

Mental Health Nurse Case Managers

Cota’s Mental Health Nurse Case Managers provide individualized support to adults (16+ years) living with a serious mental illness and complex health care needs across the City of Toronto. Only internal referrals are accepted.

External referrals are limited to St. Joseph’s Health Centre in Toronto West for ACT Step Down clients.

Consulting Psychiatrists

Cota’s consulting psychiatrists provide educational sessionals to Cota’s IST service providers who bring forward case presentations.

The consulting psychiatrists do not provide psychiatric services directly to IST clients.

Mental Health Court Support

Cota’s Mental Health Court Support Program assists adults (16+ years) with identified mental illnesses, who are involved with the justice system and have had their charges diverted, to get the treatment and support they need as an alternative to proceeding with prosecution. Services offered include referrals to/assistance with accessing short-term or long-term services in the mental health system such as case management and supportive housing.

To make a referral: Referrals are accepted directly at Cota’s Mental Health Court Support office within the 1000 Finch Avenue West Court.

Mental Health & Justice: Prevention Program

Cota’s Mental Health & Justice: Prevention Program provides individualized support to adults (16+ years) living with a serious mental illness who are at significant risk of involvement or re-involvement with the criminal justice system. Our Mental Health & Justice: Prevention Program service providers work collaboratively with their clients to provide client-centred support aimed at assisting them to minimize the probability of future involvement with the justice system and live safe, active and fulfilling lives within the community. The Cota Mental Health & Justice: Prevention Program is offered to residents of the North York Community only. Similar services are available in other Toronto communities, through other agencies.

Release Planning Case Management

The Release Planning Case Management Program is designed for inmates with a serious mental illness, about to be released from the Toronto South Detention Centre. Cota provides services to male inmates on remand (awaiting trial, sentencing or other proceedings) and offenders who are on short-term sentences. The Release Planning Case Management Program is designed to provide short-term support for clients as they transition back into the community. Individuals requiring/requesting support beyond the term of this program will be referred on to other community-based mental health services prior to discharge from the Release Planning Case Management Program. This service is available to adult males (18+ years) who are living with serious mental illness, are incarcerated at the Toronto South Detention Centre, are approaching release and are assessed by Cota as being able to be safely supported in the community through this service.

Referrals to this service are received directly through the staff at the Toronto South Detention Centre.

The Acquired Brain Injury (ABI) Behaviour Therapy Service provides behavioural support and case management services to people who are living with an ABI, are 16 years of age or older and reside within the boundaries of Toronto Central LHIN. The team which includes a psychologist, behaviour therapists and a case manager, support clients by providing a combination of skills teaching, behavioural approaches and case management supports.
